The Heroes Project was developed by the National Resource Center on Native American Aging at the University of North Dakota, and the Administration on Aging (AoA). In conjunction with the upcoming 30th anniversary of the AoA Title VI program, we are looking for your hometown heroes. We are seeking nominations from Title VI directors and other program workers for individuals who you feel have impacted the lives of elders in your community. The project seeks to honor individuals who have dedicated their time, talent, and energy to help our elders, to solve local problems, and to build stronger communities.
This project is dedicated to Alan Allery, our fallen director of the NRCNAA who passed away on July 14, 2007. He developed this idea to honor elders who work hard in tribal communities. We hope you can add your elders as a tribute.
Eligibility — Any individual in your community who has worked with your Title VI or elder’s program is eligible to be nominated. At this time, organizations are not eligible for nomination. Separate nominations may be submitted for as many different individuals as desired.
General Info — Supplementary materials may be submitted by mail (see address below).
The selection committee may request additional information and/or references from nominators for consideration in judging. Decisions of the judges are final. Because of the volume of nominations received, the selection committee cannot respond to inquiries on the status of specific nominations.
Nominees who are selected will have their stories published in AoA literature, and on the NRCNAA web site.

Key Questions
Who? Describe the nominee and his or her contributions to the Native elder community in your area.
What? Give specific examples if his or her service to the people and the scope (who or how people were reached).
When? Provide dates of service, if possible.
Where? Where did the nominee provide service?
Why? Why did or do they help? (In their words, or in your own).
Please provide a digital or high quality photo of the nominee.
