North Dakota Conrad State 30-J1 Waiver Program
The state of North Dakota participates in the administration of the Conrad J-1 Waiver program as another way to assist communities in obtaining quality health care. The Center for Rural Health and the State Health Department jointly administer this program and are prepared to consider recommending waivers of the foreign residence requirement on behalf of physicians holding J-1 visas, under certain conditions.
To apply for a waiver, a waiver package must be submitted to the Center for Rural Health for review. The completed application will be forwarded to the the State Health Department for recommendations. The Department reserves the right to recommend or decline any request for a waiver.
Three copies of the required information and documentation must be submitted to the Center for Rural Health in a single package with documents presented in the order set forth on the checklist. All documents must contain the case number assigned by the U.S. Department of State. Waiver requests that do not comply with these requirements will be returned to the submitting health care facility for revision.
