If selected as a presenter...

I understand that no honorarium or travel expense reimbursement will be provided for any presenter submitting through the "Call for Presentations" process. Oral and poster presenters receive a discounted registration fee.

I understand that as a presenter submitting through the "Call for Presentations" process all presenters are required to register as a conference attendee.

I agree to complete the presenter agreement and Continuing Education forms and submit my presentation slides and handouts by the deadline indicated in the presenter agreement. ALL SLIDES MUST BE REVIEWED BY THE OFFICE OF CME PRIOR TO THE CONFERENCE.

I understand that only a slide of my poster will be projected during my oral presentation if I am chosen to present orally.

I agree to arrange for my poster to be hung and taken down at the designated times if my poster is selected as an oral or display presenter. Failure to display during the full time may affect future ability to display.
