CHNA Implementation Plan Funding Program

The Center for Rural Health at the University of North Dakota School of Medicine & Health Sciences is pleased to announce that the State Office of Rural Health program has made funding available for North Dakota Critical Access Hospitals (CAHs), Rural Health Clinics (RHCs), and local public health units to utilize in executing activities that have resulted from their community's Community Health Needs Assessment (CHNA). The CHNA Implementation Plan Funding Program will fund a limited number of awards up to $5,000 each for North Dakota CAHs and up to $2,500 each for RHCs and local public health units. Funds are limited and will be awarded based on criteria outlined in the guidance below.
