Participation Forms

If you plan on participating in a live teleECHO™ session, you must complete these two forms and email them to Nicole Crouch.

• Intellectual Property Waiver
  To be completed by everyone (spokes and SMEs) that enter a Zoom session.
• Spoke Roster
  All attendees should sign-in at their spoke site and the site coordinator should return this form following the teleECHO session.

Forms for Presenting a Case

If you are interested in presenting a case, complete the following forms and send them to Nicole Crouch.

• Case Presentation Form
  Anyone interested in presenting a case at a teleECHO session should complete this form.
• Disclosure Form
  All presenters must complete a disclosure form. Please see the Disclosure Memo for more information.
• Curriculum Vitae (CV) Template
  If you do not already have a CV or resume created, please complete this CV template.
